Senior Care can help coordinate all day-to-day needs of a client in the home such as caregiver or companion support, transportation needs, socialization, meal management, bill paying services, financial management, and assistance with medication management. Ongoing home safety evaluations are done to review changing needs of a client. Our RNs can pre-pour medication and assist with reordering prescription refills, and supervise that clients are compliant with their medication regime.
Care managers can provide assistance with managing medical needs, including attendance at physician visits or tests, facilitation of communication between all involved health care providers, client understanding of and adherence to physician- prescribed medications and treatments, and comprehensive easy-to-understand reports to family members. SCA can also assist with medication management including pre-pouring medication, reordering prescription refills and supervising that clients are compliant with taking their medication regime.
Senior Care can visit a home, hospital room, nursing home or other facility setting to perform a comprehensive assessment of a client’s physical and mental health, functional status, environmental safety, abilities to meet daily living tasks, and financial & legal issues. Once problems and concerns are identified, we will provide written recommendations and referrals for solutions to address each need. We can then provide ongoing monitoring and support of the plan of care.
You can rest assured that we are on call 24 hours a day, 7 days a week to provide our clients with crisis intervention and support when there are unexpected changes requiring urgent decisions or immediate attention, such as emergency room visits. This provides peace of mind to our clients and especially, their family members who either live at a distance or have busy work and home responsibilities and cannot be available on short notice.
